After a reconstruction of the complex historical and epistemological development of cognitive and cognitive-behavioral therapy in infancy, the article presents the basic and shared methodological aspects of this approach and the results of the research on the effectiveness of treatments. The authors highlighthow the development of therapeutic methodology, results from the incorporation in the clinical practice of new theories of cognitive mental functioning, which gives a central role to the emotional development and interpersonal cognitive cycles. In particular, some Italian developments of cognitive psychotherapy emphasize the role of therapeutic relationship, the conception of the subject as active constructor of meaning, and the fundamental importance of the attachment relationships.

The Authors introduce the description of the various forms of parents-child therapy through a short historical overview: starting from Freud’s “kleinen Hans” they underline the importance of parents’ role for children and adolescent psychotherapists. Starting from the late sixties, the Authors describe the different models of parents-infant therapy and the therapeutic counselling for children and adolescents. The main characteristics of systemic family therapy, psychoanalytical family therapy and parental couple therapy are shown. Finally, they discuss the crucial point concerning indications for the different treatments according to the newest evidences described in literature

Psicoterapia dei disturbi d’ansia: psicopatologia, psicoterapia e outcome

The AACAP Practice Parameters for the treatment of anxiety disorders indicate that child and family psychotherapy should be considered as a mandatory part of the treatment in a multimodal approach for AD in children. This review describes different models of psychotherapy, with particular attention to efficacy and effectiveness studies. Two major theoretical models are considered, cognitive-behavioral and psychodynamic therapy, with specific treatment modalities as long as different age periods. Specific treatment components and active ingredients of these interventions are indicated.

The authors focus on the early PD organization in childhood. Developmental Psychopathology is looked upon as a telling box of the PD hystories. Borderline Personality Disorder psychotherapy evidence based studies are analysed. The importance of setting and facing a wide spectrum intervention is outlined in order to enhance the PD at risk child appropriation of his/her inner world needs in relation to others’ requests and pressures.

Il lavoro psicoterapico negli esordi psicotici in adolescenza

A review of the 2004-2009 Anglo – American (PUBMED) and European scientific literature shows that few articles focussing on “psychotherapy and brief psychotic disorders in adolescence” were published. All these articles consider immediate and intensive treatment necessary, in the form of multifocal and integrated therapy involving psychopharmacological, psychosocial, family and individual treatment. The CBT approach is the most frequently quoted type of psychotherapy and great attention is paid to working with and for families, with the aim of helping them cope with a psychotic relative. All the various cognitive therapeutic approaches attach particular attention to the relational components that are required to build a proper working alliance, and from this point of view a similarity with psychodynamic strategies can be identified. According to the literature, especially European studies, psychiatric services are effective in promoting a successful outcome if their set-up reflects a psychotherapeutic approach. In this case the different treatments provided are integrated synergistically in a strategy that aims to strengthen the potentials of patients and the surrounding environments while promoting the resumption of their personal development.

La psicoterapia del disturbo ossessivo compulsivo in età evolutiva

Several controlled studies show that cognitive behavioral therapy (CBT) is the first line treatment for children and adolescents with obsessive compulsive disorder (OCD). A substantial body of empirical and experimental literature supports exposure plus response prevention (E/RP) as the key therapy for OCD, with particularly long lasting and stable response rate, with mean symptoms reduction rate of 50 to 85%. Moreover, a cognitive conceptualization of treatment of OCD attributes the effects of E/RP to cognitive processes that produce a progressive acceptance of threat, with subsequent reduction of investment aimed at preventing it and increase acceptance to expose oneself to growing threats and risks. Nevertheless, although there is sound experimental and empirical evidence about the effectiveness of cognitive behavioral treatments with obsessive-compulsive children and adolescents, patients are not receiving them in routine clinical care.

La psicoterapia dei disturbi del comportamento alimentare

This study is a survey of the psychotherapies used in the treatment of eating disorders arising in childhood and adolescence. The current literature was reviewed using the Pub-Med and the Psycho-Info databases. 13 controlled studies published between 1999-2009 were reviewed. The theory and the technique of the main psychotherapeutic approaches are discussed. Studies provide good evidence for the efficacy of some psychotherapies.
